The 2002 Women's Health Initiative reported a relative breast-cancer increase (hazard ratio ≈ 1.26) for estrogen-plus-progestin, but in absolute terms that's roughly 9 extra cases per 10,000 women per year, and most participants were over 60, years past menopause. Longer follow-up and the "timing hypothesis" showed that women starting therapy in their 50s, within about 10 years of menopause, have a more favorable benefit–risk balance. On Nov 10, 2025 the FDA removed the boxed warnings for breast cancer, cardiovascular disease, and dementia; an endometrial-cancer warning remains for estrogen-only therapy in women with a uterus.
